Sometimes people screw up. This is a list of how people screwed up their military actions so others may learn from these lessons, or (more likely) ignore them completely.

Adhara forgot to observe the target before moving in, and also did not remember to set up backup and contingency plans when they sought to take the "freighter" in question. This obviously caused some minor problems (like a quarter of a SPF unit being vaporized).
